ivanJ4 got their new landing page design by running a design contest:
Hair Stylist App Design for Appointment Form

Cosmetics & Beauty
I have a Hairdresser Scheduling App in development and wanted to establish a relationship with a graphic designer. This contest is sort of an interview, and the goal is to put a design/UX over a form used for booking appointments. The app allows hairdressers to manager their schedule as well as share it with their clients allowing them to book directly on it.
Not looking for "fancy" styling, rather something more focused on effective UX, with visible "call to action" buttons (the Save/Cancel buttons currently show at the top). Material-type of designs are fine, but open to ideas as well. I'm attaching three images - current "design" (really just a functional implementation) and two attempts by a designer I didn't quite like. Eventually I plan to have both a light and a dark theme, but initial iteration is just a light theme, so stick to lighter backgrounds please. I'm open to the idea of using a widget library (e.g. devx) if you feel you can accomplish a good result using their components (possibly with some mix of customization).
Please see the attached examples. Currently they're shown in a "phone" format. Ideally, I would like to see examples of both, phone and tablet format (in tablet this form would basically be 2-column layout for all the form fields). The goal is to organize the form in such a way that is visually appealing and clear/effective for the user. All the functional elements are already shown in the attached document. The idea behind image uploads and all the buttons at the bottom is that when a client books an appointment directly on the hair stylist' calendar, they would upload images of their current state of grooming, and ultimately the hair stylist can approve/reject appointment. Regarding the "Services" checkboxes - the idea behind those is to be color-coded, to allow for easy bading on the main app (which is basically a calendar). So, if "RC" and "B" services were selected, those would reflect on the main app's calendar (shown in last image attachment).
