I really like the smile idea, but the font, gradient and colors make it look dated and something whimsical and unsophisticated. The "The" sitting on top of the "P" looks like they hastily put it there for lack of a better place. In addition, gradients in a logo can sometimes limit where it can be printed.
The logo should look current and somewhat simple (the best are). It should work both in the website header (most important) and shrunken down on business cards.
Once the logo is done, we will be creating a project to do the website. The website colors will be blue, orange, green and white like webmd.com:

The logo should convey that they are a promotional company, that they work with doctors, and that they are fun.
I like web 2.0 with slight gradients.
I don't like the use of capsules as a symbol for doctors as the client is anti-drug.
I am attaching the best logo so far so you can see what direction we are trying to go. The logo is not perfect, but the rest are so bad, I wanted to include this.
